Community Advocacy Program

The Community Advocacy Program (CAP), a medical-legal partnership, improves health outcomes for low-income vulnerable individuals and families including children, pregnant women, Spanish-speakers, and formerly incarcerated people. This brochure describes how CAP pairs legal and medical professionals to improve community health. It also describes how CAP trains health care providers and advocates for systematic solutions, and how you can make a referral through EPIC.
